“Ketones fight inflammation” has become one of those lines that shows up on every keto label and podcast. I get why. A 2015 paper in Nature Medicine showed that BHB switches off a major inflammation pathway in immune cells, and it got a lot of attention. What gets far less attention is what happened when researchers tried to see the same thing in people drinking ketone supplements.
I sell BHB for a living, so I have every reason to repeat the exciting version. I’d rather show you the full picture on BHB and inflammation: the cell and mouse data, the human trials that surprised people, and what it means if you use goBHB® for training or recovery.
What NLRP3 Is and Why BHB Got So Much Attention
NLRP3 is a sensor inside immune cells. Once triggered, it assembles a complex called the inflammasome, which activates an enzyme called caspase-1 and releases two signaling proteins, IL-1β and IL-18. Short term, that response is useful. Left switched on at a low level, it is linked with the chronic inflammation researchers study in obesity and aging.
In 2015 a Yale-led team (Youm et al., Nature Medicine) reported that BHB blocked NLRP3 activation in immune cells, while acetoacetate, butyrate and acetate did not. BHB also lowered IL-1β and IL-18 output from human monocytes in the lab. In mice, either BHB or a ketogenic diet reduced caspase-1 activation and IL-1β in models of inherited inflammatory conditions and crystal-induced inflammation. One detail I like: the effect did not depend on chirality, meaning both the D and L forms of BHB worked in their cell experiments.

Those were cells and mice, though. Cells in a dish can be bathed in BHB at levels a supplement may never reach in your blood, so the next step was testing people who drank a ketone supplement.
BHB and Inflammation in Human Studies
Most of the human work comes from Jonathan Little’s lab at the University of British Columbia Okanagan. They took blood before and after people drank a ketone supplement, stimulated the immune cells in that blood with a bacterial toxin (LPS), and measured how strongly NLRP3 fired.
2019: Ketone salts and ester raised NLRP3 markers short term
Neudorf and colleagues ran two randomized, double-blind, placebo-controlled experiments in healthy adults. In the first, 10 men took ketone salts at 0.3 g of BHB per kg of body weight. In the second, 18 men and women took a ketone monoester at 0.482 g of BHB per kg. Thirty minutes later, caspase-1 activation had gone up with both supplements compared with placebo, and IL-1β secretion rose in the ester study. NLRP3 and IL1B gene expression did not change. The authors warned of possible unintended effects on inflammatory activation, at least in that 30-minute window.
2020: No change in NLRP3 markers in adults with obesity
The same group then tested 11 adults with obesity in a crossover trial using the ketone monoester (482 mg/kg). Ketones did not change caspase-1 activation or IL-1β secretion compared with placebo. Plasma IL-1β dropped from baseline only in the ketone condition, which the authors said needs confirming in a bigger study.
2024: Short-lived effects in adults with or at risk for type 2 diabetes
The largest paper (Neudorf et al., 2024, Clinical and Experimental Immunology) investigated adults at risk for or living with type 2 diabetes across three studies. Adding BHB directly to blood samples lowered IL-1β and IL-6 at 5 and 10 mM (and TNF at 10 mM), but 2 mM had no effect on IL-1β. A single ketone ester drink briefly raised the anti-inflammatory signal IL-10, though that result was fragile. After 14 days of ketone ester three times a day, fasting cytokines and CRP were largely unchanged. The authors described the anti-inflammatory effect as transient. This work had public research funding from Canadian agencies.
Why the Lab Results and Human Results Don’t Line Up
Dose is the biggest part of the story. In the 2024 paper, BHB clearly calmed immune cells at 5 to 10 mM, but not at 2 mM. The ketone drinks in the trials cited here reached roughly 1.7 to 2.4 mM at peak. That gap shows up in the chart below.
BHB levels: where lab effects showed up vs. what drinks reach
Millimolar (mM) BHB. Lab rows are BHB added to blood samples; blood rows are measured after a ketone drink.

Timing is the second part. A 2021 review in BMC Medicine (Kolb et al.) notes that BHB can trigger a short pro-inflammatory signal in some cell types, followed by an adaptive anti-inflammatory response. A 30-minute snapshot after one drink could catch the wrong phase, and the longest trial here ran only 14 days.
The third part is who gets studied. Healthy adults, adults with obesity and adults with or at risk for type 2 diabetes start from very different places, and most of these trials had 10 to 18 people. Also notice that most of the human work used ketone esters, not the BHB salts in goBHB products. I covered that difference in Ketone Salts vs Ketone Esters.
Where goBHB Products Fit
I’m not going to tell you a scoop of BHB will lower your inflammation markers, because the human data doesn’t support that today. Here is how the products line up with the research.

BHB salt powders
Beyond Raw Chemistry Labs BHB Salts at GNC gives you 5 grams of goBHB per serving with calcium, magnesium, sodium and potassium. CleanForm Nutrition goBHB is a 50/50 D/L blend, 30 servings per tub, and it is NSF Certified for Sport. Both are BHB salts, the same general format as the 2019 salt experiment, though not the same product. That study gave 0.3 g/kg, about 24 grams for a 175 lb person, or roughly five servings at once.
Birds of Prey for training days

If you are using BHB around training, CleanForm’s Birds of Prey combines 5 grams of goBHB with 8 grams of L-citrulline and 5 grams of creatine and no stimulants. A 2021 review by Mansor and Woo in Frontiers in Physiology makes the case that ketones may support recovery through fuel supply and signaling, while noting the human evidence is still limited. I wrote more about that angle in goBHB and Recovery.
A safety note for all of these: BHB salts carry minerals, so a few servings add real sodium. If you watch your sodium, have kidney or blood pressure concerns, are pregnant or take medication, check with your doctor first. Our post on BHB salts side effects has the details.
Where the Evidence Stands
Cell and mouse research shows BHB can block the NLRP3 inflammasome, and both D and L forms did it in cells. Human trials have not confirmed that from a supplement. Ketone salts and esters raised NLRP3 markers 30 minutes after a large dose in healthy adults, did not change NLRP3 markers in adults with obesity, and produced short-lived changes in adults with type 2 diabetes that did not hold up after 14 days. The clear lab effects showed up at 5 to 10 mM, higher than the blood levels these ketone drinks reached. None of these products are meant to treat or prevent any disease.
FAQ
Does BHB reduce inflammation?
In cell and mouse studies, yes, BHB blocked the NLRP3 inflammasome and lowered IL-1β. In people taking ketone supplements, results have been mixed: one study saw NLRP3 markers rise shortly after a dose, and a 14-day trial saw little change in blood inflammation markers. Today, human data hasn’t confirmed it.
Do ketone salts and ketone esters work differently for inflammation?
Both raised caspase-1 activation 30 minutes after a dose in the 2019 trial. Most later human work used esters only, so there isn’t enough head-to-head data to say one form is better.
Does a ketogenic diet lower inflammation the same way?
In mice from the Youm study, a ketogenic diet reduced caspase-1 activation and IL-1β. A diet changes much more than blood BHB, though, including insulin, glucose and calorie intake. You can’t assume a supplement copies every effect of the diet.
The Bottom Line
The NLRP3 science behind BHB is real, and it is a big reason I think ketones are worth studying. The human supplement data hasn’t caught up, and in one case it pointed the other way. If you use goBHB for training or as a ketone source, use it for that, not as an anti-inflammatory. If someone sells you BHB as an anti-inflammatory, ask them which human trial they mean.
