How Long Do Ketones Last? BHB Blood Levels Hour by Hour

How Long Do Ketones Last? BHB Blood Levels Hour by Hour

One of the most common questions I get from brands and their customers is a simple one: how long do ketones last after a BHB supplement? People want to know when to drink it, whether one serving covers a whole workday, and why the meter reading they took at lunch looks nothing like the one they took at 9 AM. The answer is shorter than most labels suggest, and it depends on the form of BHB, the dose and whether you just ate.

Two human studies did the careful blood work on this: a 2017 Oxford paper by Stubbs and colleagues and a 2020 study by Cuenoud and colleagues that tracked D-BHB with blood draws and PET imaging. Below I walk through what they found, hour by hour, and how I’d time goBHB® based on it.

How Long Do Ketones Last After One Dose?

For a single serving of BHB salts, blood ketones start rising soon after you drink it, peak around the one-hour mark and are back near your starting level within 3 to 4 hours. Both studies landed on that same 3-to-4-hour window, even though they used different products and different doses.

In the Cuenoud study, groups of 15 healthy adults took 12 g of D-BHB, 12 g of a D/L BHB blend, or 15 g of MCT oil on separate days, with breakfast 30 minutes after the drink. Peak levels came about an hour after each dose, and blood BHB returned to baseline in 3 to 4 hours. Stubbs and colleagues found the same return-to-baseline window for both BHB salts and a ketone ester, though the salt took roughly twice as long to reach its peak as the ester.

Why the Form of BHB Changes the Curve

Not every ketone product gives you the same amount of BHB in the blood, even at the same dose. Here is how peak levels compared across the two studies.

Peak blood BHB by ketone form

Peak blood D-BHB in millimolar (mM) after one dose. Blue = BHB products; grey = MCT and fasting baseline. Two different studies, so compare within a study first.

Ketone ester (Stubbs 2017)2.8 mM
Pure D-BHB salts, 12 g BHB (Cuenoud 2020)1.2 mM
D/L BHB salts, matched to ester (Stubbs 2017)1.0 mM
D/L BHB salts, 12 g BHB (Cuenoud 2020)0.62 mM
MCT oil, 15 g (Cuenoud 2020)0.62 mM
Overnight fasting baseline (Cuenoud 2020)0.10 to 0.19 mM

D-BHB vs. D/L blends

Your body makes D-BHB, and standard ketone meters read D-BHB. In the Cuenoud study, pure D-BHB salts produced a peak about twice as high as the same 12 g of a racemic D/L blend (1.2 vs. 0.62 mM) and about 1.5 times the total exposure over the test. That makes sense, because half of a racemic blend is the L form, which the meter doesn’t see.

Where L-BHB goes

The L half doesn’t vanish. Stubbs and colleagues found that L-BHB from a sodium and potassium BHB salt lingered in the blood for hours after the D form had cleared, still detectable at 8 hours and gone by 24. That slower clearance is why some formulators are interested in L-BHB. It is also why a meter reading after a D/L product understates what’s in your blood. If you want the background on the two forms, I wrote about it in L-BHB vs. D-BHB: the two ketones that power the brain and body.

Esters reach higher peaks than salts at the same dose, but the D form clears on a similar 3-to-4-hour timeline. I compared the two categories in more depth in our ketone salts vs. ketone esters breakdown.

What Shortens or Stretches How Long Ketones Last

Food

Eating changes the curve. In the Stubbs work, drinking a ketone ester after a meal cut the peak by about a third (2.2 vs. 3.3 mM) compared with taking it fasted, while time to peak barely moved. The same principle should apply to salts, so a dose on an empty stomach will usually read higher on a meter.

Repeat dosing

If you want ketones up for longer, take more than one serving. Stubbs and colleagues kept blood D-BHB above 1 mM for 9 hours with a series of ester drinks. The 8-week KETAD trial (Roeth 2025), which tested BHB salts for body composition, split 10 g a day into two 5 g doses. Ketone Labs funded that trial, and its fat-loss results were trends rather than statistically significant between groups.

Dose

Bigger doses push the peak higher. Neither study tested whether they extend the window, and they add more sodium and other minerals. Less than 1.5% of the BHB showed up in urine in the Oxford data, which suggests most of it is metabolized. In a single-volunteer PET scan from the Cuenoud study, the heart and kidneys were major sites of ketone uptake.

How I Time goBHB Based on This

Think of each serving as a 3-to-4-hour window and plan around when you need it. If you want ketones high for a workout, take a D-BHB product like Beyond Raw BHB Salts (5 g of goBHB per scoop, 30 servings, $39.99 at GNC) about 30 to 60 minutes before you start, so your peak lines up with the session. For a long afternoon of work, a second serving three or so hours after the first keeps levels from dropping back to baseline.

The CleanForm goBHB Energy Shot uses L-goBHB, and the Oxford finding that L-BHB stays in circulation longer is relevant to that form. To be clear, nobody has published a blood curve for the shot itself, so I won’t promise you a number of hours. If you’re curious how researchers timed it, one Japanese trial dosed D-BHB one hour before bed and another dosed it two hours before dinner.

Where the Evidence Stands

The timing data is solid for what it covers: healthy adults, single doses, careful blood draws in two well-run studies. The Stubbs team holds commercial interests in the ketone ester it tested, and Nestlé funded and co-authored the Cuenoud work, so neither group is a disinterested party. We also have unpublished mouse data from our manufacturing partner NNB Labs (which makes BHB) comparing the timing of BHB and 1,3-butanediol products, but it is animal work, not peer-reviewed, and I wouldn’t use it to set human dosing.

What’s missing: published curves for most commercial products, including ours, and data on how training status or body weight changes clearance. If you want to see your own curve, a finger-stick meter at 0, 60, 120 and 240 minutes will tell you a lot. Our guide to testing BHB ketone levels explains how.

FAQ

How long do ketone supplements stay in your system?

For D-BHB, the form meters measure, about 3 to 4 hours after a single serving. L-BHB from racemic salts was still detectable at 8 hours in the Oxford study and gone by 24 hours.

When do BHB salts peak?

Roughly one hour after you take them, based on the Cuenoud study. Salts peaked about twice as slowly as a ketone ester in the Stubbs work.

Should I take BHB on an empty stomach?

You’ll see a higher peak if you do, since a meal cut ester peaks by about a third. If BHB salts upset your stomach, taking them with a small snack is a fair trade. See our post on BHB salts side effects for more.

How often can I take goBHB in a day?

The KETAD trial used two 5 g servings a day for 8 weeks and reported mild, placebo-level side effects. Watch your total sodium and mineral intake, and talk to your doctor first if you have high blood pressure, kidney problems, are pregnant or take prescription medication.

The Bottom Line

A single serving of BHB salts raises D-BHB for about 3 to 4 hours, peaking near the one-hour mark, and the L form lingers longer. Pure D-BHB gives a higher peak than a D/L blend at the same dose, food lowers the peak, and repeat servings are the only reliable way to keep levels up all day. Time each dose about 30 to 60 minutes before you want it working, and plan on a second serving if you need coverage past the 4-hour mark.
