L-BHB vs D-BHB: Understanding the Two Ketones That Power the Brain and Body

Ketones are finally getting the attention they deserve, but most people still misunderstand one critical detail: beta-hydroxybutyrate exists in two distinct forms, and those forms behave very differently in the human body. L-BHB and D-BHB are not interchangeable, and knowing how each one works changes how you should think about ketone supplementation altogether.
At a molecular level, L-BHB and D-BHB are mirror images of each other. They share the same chemical formula, yet the body recognizes and uses them in different ways. This is not unusual in biology. Small structural differences often determine whether a compound fuels muscle, sharpens cognition, or supports organ function.
