Do Keto Pills Work? What 3 BHB Trials Say About the Dose

Do keto pills work? I get that question from friends, customers and the occasional relative who saw a celebrity “endorsement” in a social feed. The honest answer is that the active ingredient in most of them, beta-hydroxybutyrate (BHB), has real human research behind it, but the doses that showed results look nothing like what fits in a couple of capsules or gummies.

I have spent more than two decades in supplements, and I now help run goBHB Labs, so I have a stake in BHB being sold honestly. Below is what three placebo-controlled trials found, how much BHB they used, how to read a label, and the red flags the Federal Trade Commission keeps warning about.

What Is in a Keto Pill?

Most “keto pills” and “keto gummies” are built on BHB salts, which are beta-hydroxybutyrate bound to minerals like sodium, calcium, magnesium and potassium. BHB is the main ketone your liver makes when you fast or cut carbs, and taking it as a supplement raises blood ketones without changing your diet. Some products add apple cider vinegar, green tea extract or caffeine. Many list their BHB inside a proprietary blend, which means you cannot tell how much you are getting.

That last point matters most. A size 00 capsule holds roughly half a gram to a gram of powder, and a gummy is mostly sugar or sugar alcohols set with gelatin or pectin. The trials below used 1.5 to 10 grams of BHB a day. When a bottle tells you to take two capsules, it is very likely delivering a fraction of what researchers tested.

Do Keto Pills Work? What 3 Human Trials Found

Three randomized, double-blind, placebo-controlled trials give us the best picture. None of them tested a “keto pill” brand. They tested BHB itself at known doses, which is what you should compare any product against.

KETAD: 10 grams a day of BHB salts

In the KETAD trial, published in Nutrients in November 2025, adults with a BMI of 27 to 35 followed a diet about 500 calories below their needs for 8 weeks and took either racemic (D/L) BHB mineral salts, 5 g twice daily for 10 g a day, or a maltodextrin placebo. The BHB group (24 people) lost 3.05 kg versus 1.51 kg for placebo (27 people), and 1.97 kg of DEXA-measured fat mass versus 0.82 kg. Hip circumference dropped 4.6 cm versus 0.8 cm.

Here is the caveat I always give. The between-group differences for weight (p = 0.09), fat mass (p = 0.22) and hip circumference (p = 0.05) were trends, not statistically significant results. Blood ketones were not measured, and Ketone Labs, now goBHB Labs, funded the study. The direction is encouraging, and it took 10 grams a day on top of a calorie deficit, not a pill with no diet change.

Two Japanese trials: 1.5 to 2.9 grams of D-BHB

In a 2023 trial in the Journal of Nutritional Science and Vitaminology, 44 healthy Japanese adults took 2.9 g a day of D-BHB or placebo for 12 weeks. Visceral fat area fell 9.0 cm² more with D-BHB than placebo (p = 0.037). A larger 2025 follow-up in Obesities analyzed 100 adults with high visceral fat who took 1.5 g or 2.9 g of D-BHB two hours before dinner. Visceral fat area dropped 10.6 cm² (1.5 g) and 10.9 cm² (2.9 g) versus placebo, and the 2.9 g group also lost 1.4 kg more body weight.

Both trials were modest in size, and most of the authors work for Osaka Gas or its subsidiary Osaka Gas Chemicals, which supplied the D-BHB. I covered these in more depth in our breakdown of BHB and visceral fat. Even the lowest effective dose here, 1.5 g of pure D-BHB, is more than many capsule products appear to deliver.

How to Read a Keto Supplement Label

Start with the number. Look for BHB listed in grams per serving, not buried in a blend, and compare it with the 1.5 to 10 g range above. Next, check the form. D-BHB is the form your body makes and the one standard blood meters read, while racemic D/L salts contain about half L-BHB. Both have human data, at different doses.

Then check the minerals. BHB salts carry sodium and other minerals, so a 5 g serving can add a couple hundred milligrams of sodium. Finally, look for third-party testing such as NSF Certified for Sport, and for the branded ingredient name. Products made with goBHB® list it on the label, which also matters because Axcess Global Sciences, which licenses the goBHB patents, won a $2 million trademark counterfeiting judgment against a counterfeiter (details here).

Products That Show Their Dose

I am biased here, so I will stick to label facts. GNC’s Beyond Raw Chemistry Labs BHB Salts has 5 g of goBHB D-BHB per serving with 115 mg calcium, 135 mg magnesium, 210 mg sodium and 127 mg potassium, unflavored. That one serving is more D-BHB than either Japanese trial used, and the same gram amount as one KETAD dose, though KETAD used racemic salts. Sports Research Keto+ has 6 g of goBHB salts in a 10.5 g scoop, plus 600 mg leucine, 250 mg acetyl-L-carnitine, 250 mg lysine and 100 mcg chromium, at 20 calories.

Neither is a pill. Getting to a researched dose takes a scoop of powder. For the full list of products that use our ingredient, see the 2026 goBHB products guide.

Red Flags the FTC Warns About

The Federal Trade Commission has warned consumers about fake Shark Tank endorsements used to sell weight loss products and keto diet pills, complete with doctored photos and videos. Its advice is to search the product name with words like “scam” or “complaints” and to check the official source instead of following links in ads. A separate FTC alert lists claims that are always a scam, including that all you have to do is take a pill, or that you don’t have to watch what you eat.

My own list adds a few: no BHB amount on the label, “before and after” photos that look like stock images, a “free trial” that asks for a card, and any promise of pounds lost per week. A real BHB product tells you exactly how many grams you are getting and does not need a celebrity to sell it.

Where the Evidence Stands

Three placebo-controlled human trials suggest BHB, at 1.5 to 10 grams a day, may help support reductions in body fat, especially visceral fat, alongside a sensible diet. The effects are modest, the trials are small and were funded or run by the companies that make the ingredient, and the KETAD body-weight results were trends rather than significant differences. No trial has tested the typical low-dose keto capsule or gummy. None of these products are meant to treat or prevent any disease. If you take medication for blood pressure, have kidney concerns, or are pregnant or nursing, talk with your doctor before adding mineral-bound BHB. Some people get stomach upset at higher doses, so start with one serving.

Frequently Asked Questions

Do keto pills actually help you lose weight?

Not at the doses most capsules provide. The trials showing modest fat loss used 1.5 to 10 grams of BHB a day, and the 10 g trial combined it with a calorie deficit. A couple of capsules usually holds far less than that.

How much BHB should be in a keto supplement?

Look for at least 1.5 g of D-BHB or several grams of D/L BHB salts per serving, listed clearly on the label. Research doses ranged from 1.5 g of D-BHB to 10 g of racemic salts a day.

Are keto gummies a scam?

Not every one, but the category is full of fake celebrity endorsements and subscription traps, which the FTC has warned about. A typical gummy is unlikely to deliver a research-level dose of BHB, so check the label and buy from a known retailer.

Do you need to be on a keto diet for BHB to work?

No. The KETAD trial used a moderate “Zone”-style diet with about 40% carbohydrate, so participants were not on a ketogenic diet. Even so, the placebo group on the same deficit lost about half as much weight as the BHB group, so diet did a large share of the work.

The Bottom Line

So, do keto pills work? BHB has promising human data for body composition, but at doses that do not fit in a capsule, and none of the trials tested the products sold through celebrity ads. If you want to try BHB, choose a product that lists its grams per serving, match it to the research, and pair it with a calorie deficit.
